A Man With Intractable Convulsion

Tetanus is a rare disease in the United States. Fewer than 40 cases are reported annually because of the high incidence of vaccination. Recognition of the clinical presentations is important because laboratory recovery of pathogen is only 30%, and toxin detection is rare because of consumption at motor neurons. We report a case of tetanus in an elderly man who had a reaction to tetanus vaccination as a child and was nonvaccinated through adult life.

We report a case in which a 42-yr-old man suffered a unilateral convulsion immediately after i.v. injection of propofol, and was discovered subsequently to have an old contralateral cerebral infarct. This complication and the current information on the relationship between propofol and abnormal neurological activity are discussed.

Nine patients with transient hemiparesis following a febrile convulsion were part of a group of 71 patients undergoing temporal lobectomy with mesial temporal sclerosis or non-specific changes in the specimen. There was no gross evidence of additional brain damage in tests before surgery and their response to surgery was as good as the 62 patients without this history.
